We hosted a very well attended meeting for our 53 national partners in London on 14 June 2018 with a number of excellent presentations from our invited speakers.
C4CC co-chairs Anna Severwright and Nigel Mathers set the scene with their introduction before Nicola Kay, Deputy Director of the Personalised Care Group, shared NHS England’s Comprehensive Model for personalised care with a striking new animation which is due to be launched soon. C4CC partners were supportive of the NHS future vision for personalised care and stressed a ‘People’s pathway for personalised health’ is necessary to compliment the system change.
To mark Carers Week, Richard Cross from our co-production team gave a powerful insight into his personal experience and the challenges faced by carers on a daily basis.
Sheffield GP Dr Ollie Hart then took the opportunity to present and seek feedback on his quadrant model – an innovative new person-centred approach to long-term condition management in primary care. The presentation was described by one attendee as “the best articulation of patient activation I have ever heard”. A blog post and papers on this new model can be accessed here.
The final presentation came from Jacqueline Del Castillo of Nesta and Tom Harrison of the RSA on health as a social movement.
